I, ________________________________________ , hereby authorize the current Beta Alpha Psi - Epsilon Chi Chapter Faculty Advisor, President and Vice President of Membership access to the information contained in my Degree Progress Report (DPR) and Academic Transcript.

        I understand that the information contained in my DPR and Academic Transcript are protected under the Family Education Rights and Privacy Act (FERPA) of 1974. I authorize the Faculty Advisor, President and the Vice President of Membership to use the academic information within these documents to verify my Grade Point Average (GPA) as part of my application to be eligible for membership with the Epsilon Chi Chapter. I understand that the following information will not be used outside its authorized purpose, and I have the right to submit a formal complaint to the Faculty Advisor, the Accounting Department Chair and/or the Dean of the David Nazarian College of Business and Economics if the information is used beyond the scope of my authorization.
